Everybody (if not previously submitted) complete, initial and sign COVID WAIVER

New Athletes: read, sign all the following forms, with a copy of birth certificate (we keep); athlete signs middle of contract and the code of conduct (also, we don’t need a physician note unless athlete has a medical issue, otherwise parent is certifying athlete has no medical or physical condition)

Returning athletes: need only to complete the Volunteer, Information and Uniform Order

PLEASE NOTE: This year, we have YXS size-it is not on the form, so please write it in. Also, the volunteer form has not been revised: this year, volunteer deposit is $100 against 10 hours of volunteer time at meets.
